Pentamidine Isethionate Injection

Pentamidine isethionate an antiprotozoal agent used to treat infections such as leishmaniasis and African trypanosomiasis, mainly in dogs, and to prevent Pneumocystis pneumonia, a serious lung infection seen in animals with weakened immune systems. Crucial in managing infections that are resistant to first-line treatments.

Pentamidine is an antiprotozoal agent used primarily in dogs, and exotic animals like ferrets.

INDICATIONS

  • Trypanosomiasis: Used for the treatment of infections caused by Trypanosoma brucei (African sleeping sickness). Studies in dogs indicate it effectively clears parasites from the blood, often showing superior action compared to other agents like diminazene.
  • Leishmaniasis: Indicated for both cutaneous and visceral leishmaniasis (VL), including Canine Leishmaniasis (CanL). It helps move the disease toward an asymptomatic state and can help recover cellular immune responses.
  • Pneumocystis Pneumonia (PCP): Used for the treatment of pneumonia caused by Pneumocystis carinii (also known as P. jirovecii). It is specifically documented for use in ferrets at dosages such as 3–4 mg/kg.
  • Babesiosis: Some pharmacological reviews list it as a compound with activity against Babesia species in animals. 

ADMINISTRATION

  • Intramuscular (IM) Injection: This is a common route for veterinary use, such as treating trypanosomiasis in dogs. It should be administered as a deep IM injection into a large muscle mass.
  • Intravenous (IV) Infusion: Pentamidine can be given intravenously, but it must be administered as a slow infusion over 60 to 120 minutes. Rapid IV bolus must be avoided, as it can cause life-threatening hypotension or collapse.
  • Inhalation (Aerosol): While less common in general veterinary practice, pentamidine isethionate can be administered via a nebulizer for localized lung treatment or prophylaxis (e.g., in experimental models like rats or dogs). 
Dosage in dogs
A common treatment regimen for trypanosomiasis involves intramuscular injections of 4 mg/kg of body weight once daily for seven consecutive days. Dosages can vary based on the specific condition being treated and the animal’s response.
